I would like to here thank all those who took the time and trouble to fill out the on-line questionnaire. The questionnaire is being hosted and managed by Harry Bosma in the Netherlands and I am very grateful for his expert help and kindness.
I also wish to thank Martin Gasser and Christian Gfeller of the Paul Klee Center for CMS help and, more recently, Andreas Schlapbach who migrated the website to a more recent version.
I have had two persons who have provided immense help by translating the early French papers into English (Eve Portmann) and into German (Dr. Maurice Schommer) and I am very grateful for the excellent work they did.

I wish here to express my gratitude to Kei Ito for all his help in the initial stages of this website and with the data analysis. As far as I am aware, Kei was the first researcher to put a déjà vu questionnaire up on the World Wide Web.
